However, he is also after Ghost's own Eyecons and stops at nothing to steal them so he can have his own wish granted: to bring his sister's body back from the Gamma's dimension. He even aligns himself with Chikara Saionji and Alain to reach his goal, and fights Ghost and the Gammas to get the missing Eyecons. However, Saionji is merely using him to collect the Eyecons for him, and when all 15 are gathered, and Kanon is kidnapped by Saionji and the Green Dragon Blade Gamma, forcing Makoto to do his bidding. Takeru confronts him and promises that he will save Makoto's sister, breaking the other man's cold exterior to realize despite everything, Takeru still considers him a friend. After Kanon is saved, Makoto promises to help Takeru find the Eyecons once more so he can return to life.

Makoto plans to return to the Gamma World on his own, feeling that he has unfinished business within it, and leaves Kanon in the care of his friends. However, while fighting the Knife Gamma, he is attacked by Alain who then uses his Necrom Eyecon in Makoto's Ghost Driver, making him his brainwashed servant. After apparently killing the Art Supplies Gamma, Makoto manages to break free from the spell with the help of Kanon, Takeru, and the spirits of Tutankhamun, Nobunaga, and Houdini. It is revealed that Makoto's soul was removed from his original human body, which is kept by Igor in his lab, and transferred into a Gamma body.
As a result, Makoto can freely travel between the human and Gamma worlds. During his return to the Gamma World to get his body back, he visits Alia, thanking her for raising him and Kanon, but she tells him that Alain has been framed by Adel for killing Adonis. After entrusting his Machine Hoodie, heroic Eyecons and Alain to his friends, Makoto apparently lets himself be killed by Javel. However, this is all a plan to get his real body back. After his Gamma Eyecon body is destroyed, he wakes up in a stasis capsule and retrieves his Specter Eyecon from Edith's Eyecon storage before returning to the human world.
Edith also gives him the Deep Specter Eyecon, but warns him to be wary of its dangerous powers. He eventually manages to catch up with Takeru and Alain, where Takeru informs Makoto that Alain has become inconsolable after he witnessed Adel kill Adonis. Makoto snaps Alain out of it by punching him in the face before telling him that he still has his father's good will to carry on. Sometime later, he faces off against a dopplegänger of himself who presumably works under Adel's servitude. Almost an entire army of dopplegängers were created as a side effect using the Deep Specter Eyecon. These clones all appear in Specter Damashii, but were almost as strong as Deep Specter. Despite easily killing them, he suffered a fatal chest pain from each clone's destruction.
During a body switch incident between detective Shinichirō Natsume and his teenage daughter Mayu Natsume that was caused by Gammiser Magnetic Blade, Makoto is reminded of the way Mayu hates her father the same way he hated his long-lost biological father Daigo for not caring about him and Kanon ever since their mother died, which is the reason why Makoto looked up to Takeru's father Ryū as more of a father figure than Daigo. In Kamen Rider Ghost the Movie: The 100 Eyecons and Ghost's Fated Moment, he finally found his father, Daigo to settle their conflicts. Though he beat his father, proven to him that Makoto has now grown stronger, their battle is interrupted by the appearance of Dark Ghost who takes the Darwin Eyecon from Daigo.

After Specter's unlocking of the OOO Eyecon to defeat the Kyoryu Greeed leaves only one more to go, the scenario is interrupted by Xibalba, with the two Riders realizing that they were meant to destroy this monster as it absorbs Frey and Freya, enhancing its Gamma Ultima body.

Greatly overwhelmed, Specter barely holds off Xibalba as Ghost manages to unlock the Wizard Eyecon, defeating Wiseman. With all six Rider Eyecons in their hands, the two Riders use them as per intention, going through the Rider Damashii they have accessed to fight Xibalba, managing to free Frey and Freya as they wear him down to his original form before destroying him with a combined Omega Drive using the power of the Rider spirits. Ultimately, Makoto and Takeru get no answer as to who exactly Freya and Frey are, but have learnt through collecting the Rider Eyecons that they can overcome many challenges and that if any villain plans to make their world despair.

Later on, the Riders are led by the Rider Eyecon to the advent of the last enemy, a Gamma Ultima known as the Dark Mind who brings with him revived Nova Shocker generals Urga and Buffal. Backing Ghost up, Specter witnesses him make use of the Kamen Rider 45 Ghost Eyecon, first accessing Ichigou Damashii to destroy Urga and Buffal before transforming into the combined Heisei Damashii to defeat the Dark Mind even after he enhances his form with the Alexander Gamma Eyecon. Their Rider quest finally over, the two Riders delight in their victory.

An iteration of Kamen Rider Specter fought in the Game World's Chou Super Hero Taisen tournament as part of Kamen Rider Genm's Team Genm, competing for the title of Kamen Sentai Gorider with Specter serving as candidate for Aorider. Team Genm ultimately made it to the final round, however, while Specter defeated AoNingerIcon-crosswiki, Team Genm was beaten by Team Ex-Aid as Ex-Aid bested Genm.

Damashii
Specter's forms are called Damashii, accessed using the Ghost Eyecons. He also bears a headpiece called the Persona, Latin for "Mask", which gives off a glow effect with the rest of his body. The Persona changes with Specter's numerous Damashii forms, as well as serving as a prefix title for all of the Damashii forms' helms. Differentiating himself from Kamen Rider Ghost, Specter normally bears a pair of Wisp Horns on his head in most of his Damashii forms, allowing him to use them either for perceiving and sensing Gamma within a 8 km radius or as a weapon. When transforming into Houdini Damashii, the Wisp Horns are upgraded into the Striker Horns, which upgrades its sensory range with an additional 4 km. to a total of 12 km. of range.
When transforming into Deep Specter, the Wisp Horns are upgraded into the Chaos Wisp Horns. While they don't offer an expansion to the sensory abilities, it appears the Chaos Wisp Horns are capable of releasing the pent up energy within Deep Specter as a form of plasma, which can be weaponized as a form of attack. By pulling and pushing the Ghost Driver's lever a second time after transforming, Specter can activate an Omega Drive finishing attack tied to the Ghost Eyecon currently being used. If he pulls and pushes the lever four times, he can instead execute an Oomedama, "Giant eyeball" special attack.

- Specter Damashii is Specter's default blue form. Accessed through the Specter Ghost Eyecon, this form bears the Persona Berserk helmet with the Face Berserk faceplate. This form debuts in episode 4. While assuming Specter Damashii, Specter dons the Berserk Reflector.
- In this form, Specter has much more power than Ghost has in his Ore Damashii. He also has more agility than Ghost, making Specter overall is more prowess than Ghost. In a similar vein to Ghost's anti-gravity levitation, Specter can utilize superhuman speed, which commonly used as an escape from the enemy or simply a method of transportation.
